What Is Shoulder Impingement Syndrome?

Shoulder impingement occurs when your rotator cuff (a group of muscles and tendons surrounding the shoulder joint) rubs against the top of your shoulder blade (acromion), causing swelling and pain.

What Are the Symptoms?

The common symptoms of shoulder impingement include:

  • Sharp pain when you lift your arm
  • Soreness in the front of your shoulder and side of your arm
  • Popping sensation when moving your shoulder
  • Shoulder stiffness

What Causes It?

  • Overuse of the shoulder
  • Injury
  • Wear and tear due to age

How Is It Diagnosed?

Dr. Ong will conduct a physical examination of your shoulder and arm to test their range of motion and check for any pain or tenderness.

Imaging tests such as x-rays can detect abnormal bone changes or bone spurs that may cause the impingement. X-rays can also eliminate other joint problems such as arthritis. Further screening tests such as an ultrasound or MRI scan may be conducted to have a clearer view of your shoulder and confirm any rotator cuff tendon tear.

What Are the Treatment Options?

There are various treatment options available, depending on the severity of your shoulder impingement. The goal of treatment is generally to alleviate pain and restore shoulder mobility.

Non-Surgical Options

Not all shoulder impingements require surgery.

In minor cases, your shoulder specialist will recommend the following options:

  • Home remedies. Resting your shoulder by avoiding excessive movements and strenuous exercises can prevent further shoulder damage.
  • Physical therapy. Dr. Ong may recommend simple stretching and strengthening exercises to allow your shoulders to regain their full range of motion and mobility.
  • Medications. Taking over-the-counter pain relievers such as paracetamol, aspirin or ibuprofen can also help to reduce pain.

Surgical Options

For more severe cases of shoulder impingement, surgery may be recommended.

A minimally invasive arthroscopic surgery to remove part of your acromion is usually performed. This surgery widens the space around your rotator cuff and minimises friction between your acromion and rotator cuff.

However, traditional open surgery may be required for more advanced cases.

Dr. Ong Kee Leong is a fellowship-trained orthopaedic surgeon. He subspecializes in shoulder and knee, foot and ankle, hand wrist and elbow surgeries, arthroscopic sports surgery, and the management of sports-related injuries.

He has been registered with the Singapore Medical Council as a specialist in Orthopaedic Surgery since 2011.
