What Is an Ankle Ligament Tear?

Ankle ligament tear, commonly known as an ankle sprain, is a rupture of the ankle ligaments, causing swelling and pain. It is one of the most common conditions treated by a foot and ankle specialist.

What Causes It?

  • Falls from walking or running, which can twist the ankle
  • Direct hit
  • Sudden changes in movement when pivoting or jumping

How Is It Treated?

  • RICE approach
    • Rest. Restrict your movement and avoid activities that cause pain to your ankle.
    • Ice. Applying cold compression to your sprained ankle can alleviate swelling and pain.
    • Compress. Wrapping an elastic bandage firmly around your sprained ankle helps to immobilise the ankle and reduce swelling.
    • Elevate. Elevating your ankle above your heart level can minimise swelling by reducing blood flow.
  • Over-the-counter pain relievers
  • Physical therapy, which includes gentle stretching and strengthening exercises
  • Surgery. Surgery to repair or reconstruct a ligament may be required if symptoms persist or the ankle remains unstable despite non-surgical treatment options.

What Are the Symptoms?

  • Tenderness
  • Snapping or popping sensation
  • Bruising
  • Swelling of the ankle
  • Restricted ankle mobility
  • Ankle instability
  • Sharp pain, especially when you walk or bear weight on the injured foot

How to Diagnose It?

A physical examination will be conducted to test your ankle’s range of motion and stability via several simple motion exercises.

Further imaging tests such as an x-ray, MRI scan or ultrasound may be performed to rule out other possible causes of pain (such as a bone fracture) and evaluate the extent of ligament damage to confirm the diagnosis.

Dr. Ong Kee Leong is a fellowship-trained orthopaedic surgeon. He subspecializes in shoulder and knee, foot and ankle, hand wrist and elbow surgeries, arthroscopic sports surgery, and the management of sports-related injuries.

He has been registered with the Singapore Medical Council as a specialist in Orthopaedic Surgery since 2011.
